Welcome to Preferred Care Hospice

At home with compassionate care.

Preferred Care Hospice is here to help patients make choices that work for them and provide a support system of comprehensive care to honor those choices.

We are a team of doctors, nurses, social workers, nurses’ aides, counselors, chaplains and volunteers who have extensive training and are certified in hospice and palliative care.

Although many patients admitted to hospice care have cancer, there are numerous terminal diagnoses that qualify for the hospice benefit.

We’re equipped to work with patients of many ailments and situations and are happy to assist with each family’s time of need.

This includes end stages of:

• Cancer
• Dementia
• Heart Disease/CHF
• Liver Disease
• Renal Disease/No Dialysis
• Pulmonary Disease/COPD
• Neurological Disease/ALS/Parkinson’s/MS/MD
• AIDS/HIV
• Protein Marasmus
• Stroke/Coma
Patients do not have to be bedridden to be eligible for hospice care. Preferred Care Hospice encourages patients to be physically active as possible, for as long as possible.

We believe in continued interaction with your primary care physician. The visits, comfort, knowledge, and support from your family doctor can be very helpful.

Preferred Care Hospice focuses on the quality of life with family-centered care in the comfort of your own home, hospital, nursing home or assisted living facility. Anywhere the patient prefers.

If you’re interested in learning more about your situation and whether Preferred Care Hospice can help, contact us today; we’re happy to discuss a personalized care plan to help meet your needs to the best of our abilities.

We are here to help.

Preferred Care Hospice affirms life and does not hasten or postpone passing from this life. We treat the person, not the disease. Our focus is on quality of life, involving the patient and family in the decision making-process.

Our goal is to enable you to live in your own home, together with your family. We work to help alleviate symptoms while providing comfort and promoting an opportunity to bond emotionally and spiritually with our patients.
